Part Time HIV/AIDS Registered Nurse

The Pratt Area Community Council (PACC) is a not-for-profit, membership-based community development corporation entering its fifth decade. We combine tenant and community organizing, tenant and homeowner services, affordable housing development and management, and economic development to improve the Central Brooklyn communities.

PACC is seeking a registered nurse who is experienced in working with people who are living with HIV/AIDS, mental illnesses, illegal substance use and other chronic diseases. The registered nurse, reports directly to the Associate Director of Social Services, will be responsible for addressing the basic medical needs of 50 residents. He/she must be experienced in a supportive residential housing program, understands the harm reduction method, and be supportive of its members. He/she must be able to work well in a multi-disciplinary social services environment.

RESPONSIBILITIES:

  • Provide medical care to 50 residents many of whom are living with HIV/AIDS, mental illnesses, and illegal substance use.

  • Administer and dispense medication to patients as needed.

  • Triage and care for acutely and chronically ill residents.

  • Participate in the client interview process to assess level of medical needs and design treatment plan.

  • Conduct relevant medical in-service training to the staff members of PACC

  • Obtain patient’s medical and psychosocial history.

  • Collect and maintain patient data and record.

  • Serve as liaison between patients and their mental health providers and primary care physicians.

  • Assess patients’ needs and conduct appropriate follow-up medical care

  • Work collaboratively with residents’ PCPs and assist them in adjusting medication to lessen or avoid medical side affects.

  • Assist residents in complying with medical adherence.

  • Ensure the agency’s medical component comply with funder and state’s requirements.

REQUIREMENTS:

Candidate must be licensed and registered with the New York State Medical Board. He/she must have at least three years of experience working with the people who are living with HIV/AIDS and the disfranchised population. He/she must be passionate and committed to the agency’s mission and willing to help the residents achieve the ultimate goal of independent living.

Hours: Flexible schedule of a three-day work week, seven hours each day.
